APC House follows a clear, structured care process designed to support safety, stabilization, and continuity of care. We work closely with placement agencies, families, and professionals to ensure each youth receives appropriate services from referral through transition.

Referral/Inquiry

Placement agencies or referring parties submit a referral with basic youth information and relevant records for review.

Review & Intake Planning

APC House reviews the referral to assess service needs, safety considerations, and program fit. Intake planning is coordinated with the placing agency to ensure appropriate placement.

Admission & Orientation

Upon admission, youth receive orientation to house expectations, daily routines, and available supports to promote a smooth transition into care.

Stabilization & Skill Building

Youth participate in structured daily routines, therapeutic services, and behavior support strategies focused on emotional stabilization, coping skills, and personal growth.

Family & Agency Communication

APC House maintains consistent communication with placement agencies and families through documentation, progress updates, and coordinated planning.

Transition & Next Steps

As youth progress, APC House supports transition planning, including aftercare coordination, reunification planning when appropriate, or preparation for the next placement.
